Referring a Patient to Clarity Rehabilitation

Thank you for considering Clarity Rehabilitation for your patients. We know that finding reliable, high-quality outpatient speech therapy for adults with complex neurological diagnoses is not always easy, and we built this practice specifically to fill that gap.

Clarity Rehabilitation provides in-home outpatient speech-language pathology services throughout the Denver metro area. We serve adult patients in their own homes, in assisted living facilities, and in independent living communities. Our practice focuses exclusively on adult neurological rehabilitation, which means the patients you refer to us are seen by a clinician who has dedicated their career to exactly these diagnoses.

Who We Treat

We welcome referrals for adult patients (18 and older) across a broad range of neurological and medical diagnoses. The populations we see most frequently include:

Stroke and Acquired Brain Injury: Aphasia (expressive, receptive, or mixed), dysarthria and apraxia of speech, dysphagia and aspiration risk management, and cognitive-communication disorders following stroke or traumatic brain injury.

Parkinson's Disease: Hypokinetic dysarthria, hypophonia, dysphagia (including silent aspiration), cognitive-communication changes, and voice disorders characterized by reduced loudness and vocal effort.

Neurodegenerative Diseases: ALS, with management of speech, voice, and swallowing across the disease course. Dementia, including Alzheimer's disease, Lewy body dementia, vascular dementia, and frontotemporal dementia, with a focus on functional communication and caregiver training. Primary progressive aphasia. Multiple sclerosis, including cognitive-communication concerns and dysarthria.

Other Neurological and Medical Presentations: Post-COVID cognitive and communication changes, voice disorders of neurological or structural origin, mild cognitive impairment with strategy-based rehabilitation goals, and post-hospitalization presentations affecting communication or swallowing function.

Our Service Model

Clarity Rehabilitation is an outpatient speech-language pathology practice. We are not a home health agency. This is an important distinction for two reasons.

First, your patients do not need to qualify as homebound to receive our services. Any adult patient for whom in-home therapy is the most appropriate or accessible setting is eligible to work with us, regardless of their functional mobility.

Second, our services are billed under Medicare Part B outpatient therapy. This means all standard outpatient billing practices, documentation requirements, and patient rights that apply to clinic-based outpatient therapy apply equally to our services.

For patients transitioning out of acute care, inpatient rehabilitation, or skilled nursing, we are well positioned to provide continuity of outpatient-level care without requiring patients to travel for ongoing therapy. Treating patients in their home environment also allows us to work directly with caregivers, assess real mealtime setups for dysphagia patients, and practice communication strategies in the natural contexts where they matter most.

Insurance and Billing

We accept Medicare Part B and are in network with select private insurance plans. Private pay is also available. We handle insurance verification and benefits coordination on our end, so your office will not be asked to manage the billing relationship on a patient's behalf. If you have questions about whether a specific patient's insurance is accepted, please contact us directly and we will verify before you submit the referral.

How to Refer a Patient to Us

Step 1: Send us the referral. You can refer by phone, fax, or email. We ask for basic demographic information, the patient's primary diagnosis, and a brief description of the presenting concern. A formal physician's order is not required to initiate a speech-language evaluation under Medicare Part B. We can begin the intake process from the referral alone and coordinate any necessary documentation with your office afterward.

Step 2: We reach out to the patient. Within one to two business days of receiving the referral, we will contact the patient or their designated caregiver directly. We will introduce ourselves, conduct a brief phone consultation to understand their needs, verify insurance eligibility, and schedule the initial evaluation at a time that works for them.

Step 3: Evaluation and plan of care. We complete a comprehensive speech-language evaluation in the patient's home. You will receive a written summary of our findings and the recommended plan of care shortly after the evaluation is complete. We will flag any clinical concerns that we believe warrant your attention or follow-up.

Step 4: Ongoing communication. We provide progress updates at clinically significant intervals throughout the patient's course of care. We are available by phone or email for any questions about a shared patient. Our goal is to function as a collaborative and communicative extension of your patient's care team.

Discharge Planners and Case Managers

We understand the time pressure that comes with discharge coordination, and we want to make your job easier. If you are planning a discharge from an acute care hospital, inpatient rehabilitation facility, or skilled nursing facility and are looking for an outpatient speech therapy option that does not require your patient to travel, please reach out.

We will respond promptly, be straightforward about our availability, and keep you informed throughout the intake process so you can close the loop on your end with confidence.
